REFERRING VETERINARIANS


No need to fill out a new form!

Once you submit a form, you should receive a confirmation email to the address you provided for yourself or clinic. In that confirmation email, scroll to the bottom and find a link to “edit form”. After you make corrections and resubmit, a new confirmation email will be sent to you and to the Central service you selected.

We view ourselves as complementing your practice and serving as an extension of your clinic. With that in mind, we strive to keep you involved in your referral cases as much as possible. At the core of that effort is the delivery of timely medical and diagnostic results. After all, the pet is your patient and we realize that you have a need to know whatโ€™s going on in order to keep your client fully informed.

We also want to be proactive and interactive, and are always looking to make improvements on our end in the referral process. If thereโ€™s something we can do differently or better that would help make your job easier or more efficient, please let us know. We believe having an open and honest relationship is the best way to be productive in business.
